Breast implant safety is a subject of growing international concern, and at the center of the controversy is a rare form of cancer that occurs most often in implants with rough surfaces known as textured implants. The International Consortium of Investigative Journalists revealed the ongoing health problems plaguing women with breast implants as part of its global Implant Files investigation in November , and has covered breast implant safety extensively in the aftermath.

In the wake of the investigation, health authorities from France to Brazil to the United States recently announced initiatives to better protect patient health. With public concerns increasing and major decisions on breast implant safety expected soon from regulators in several countries, ICIJ answers the key questions about textured implants and their safety.

Textured breast implants have a rough surface that is sometimes compared to sandpaper. Unlike smooth-surfaced implants, their surface adheres to the tissue that surrounds them, preventing them from moving around within the implant pocket created by the surgeon. Studies have shown that patients with textured implants face a higher risk of a rare form of cancer called breast implant associated anaplastic large cell lymphoma BIA ALCL.

Food and Drug Administration issued its first report on the disease. Saline implants are less clear-cut however. Insurance generally will not pay for removal of a ruptured saline implant. This could be taken to mean ruptured saline implants do not cause problems. And anecdotally this appears to be true. But I still recommend removing the implant if it is ruptured. But if the breast is hard or painful, most women will want to have the implant and capsule removed simply for comfort.

Breasts change over time, whether you have implants or not. Weight gain or loss will cause breasts to change size. Pregnancy and breastfeeding also cause changes in breast size and shape. And over time breast tissue shrinks and skin stretches, causing ptosis i. Breast implants can increase the amount of stretch on the skin, but the degree to which this happens depends on the size of the implant. The larger the implant, the more it will stretch the skin.

Why do breast changes over time matter? For two reasons. First, your breasts appearance will change as you age. Your breasts may drop lower that pesky droopiness again! Or as the tissue thins the implants may become more visible in the form of rippling. Second, if or when you have your breast implants removed, your breasts will look different than they did prior to breast augmentation. The changes are a combination of skin stretch from the implant and the natural changes breast undergo over time.

Many women are totally fine with this. Others opt to have a breast lift aka mastopexy done at the time the implants are removed. We covered a ton of information in this blog post. You have a few options for breast implant removal. You and your plastic surgeon can decide on simply removing the implants, removing the implants while also performing a breast lift, removing the implants and hardened scar tissue that has formed or an implant replacement with or without a breast lift. Your plastic surgeon will discuss your options based on your goals, anatomy, and existing breast implants in order to help you choose what will work best for your desired results.

Proper care for your breast implants can help to improve their longevity, and this care begins during your breast augmentation recovery. For example, your plastic surgeon will advise you to wear a support garment or surgical bra during recovery. If you do not follow these guidelines, the lack of support can lead to increased sagging over time. You should always follow your plastic surgeon's guidelines for annual checkups, as well as performing regular self-checks. If you have silicone implants, the FDA recommends getting an MRI three years after the initial surgery and then every two years after that.

Taking proper care of your implants can help you extend the life of your new look and give you more time before you are likely to require breast implant replacement.
